Oh yes, this is definitely an issue for most people! The good thing for me is that I've never had any clients. As in I was never freelancing or working for anyone else but me. But of course, I need to manage teams of people and to make sure that everything's going smoothly, so going away for long periods of time is generally quite difficult to arrange.
When going on those trips, I try to make sure that I'll be able to get some connection at least 1-2 times per day (during which I answer all of my e-mails and messages to make sure that things are running smoothly). If I'm going to be completely offline (like when I was in North Korea), then I communicate that in advance and make sure that someone else (generally a partner of mine) can take care of my tasks while I'm gone.
